Pick up a turmeric supplement and the front of the bottle probably says something like 1,500 mg. That number feels generous. It is also close to meaningless.

Turmeric root is only 1 to 6 percent curcuminoids by dry weight, according to the NIH LiverTox database. Curcuminoids are what almost every study is actually about. So 1,500 mg of plain turmeric root delivers somewhere around 45 mg of the thing you are buying it for. Clinical trials typically use ten to thirty times that.

The short answer: the best turmeric supplement tells you how many milligrams of curcuminoids you are actually getting, not just how much turmeric powder is in the capsule. Look for a standardised extract at 95 percent curcuminoids, an absorption aid such as piperine or a lipid based delivery system, and independent testing for heavy metals. Everything else is packaging.

Turmeric or curcumin? They are not the same purchase

This is the confusion the whole category is built on.

Turmeric is the root. It is a spice, it is mostly starch and fibre, and its yellow colour comes from a small group of compounds called curcuminoids.

Curcumin, more accurately curcuminoids, is that small active fraction. Nearly all the research everybody cites was done on concentrated curcuminoid extract, not on turmeric powder.

So a label saying 1,500 mg turmeric and a label saying 500 mg curcumin extract are describing completely different things, and the bigger number is the weaker product.

Look at rows one and three. The 100 mg capsule delivers roughly twice the curcuminoids of the 1,500 mg one. That is the entire trick of turmeric marketing in a single comparison.

The last row is what most shelves actually look like. No percentage means no calculation, which means no comparison, which is usually the point.

Does black pepper actually help? Here, yes.

We have been sceptical of absorption claims elsewhere in this blog, so it is worth being equally clear when the evidence holds up. For curcumin, it does.

Curcumin has a genuine absorption problem. It dissolves poorly in water, and what does get absorbed is broken down very quickly by enzymes in the gut wall and liver. Take it on its own and most of it never reaches your bloodstream.

Piperine, the active compound in black pepper, slows those specific enzymes down. In a 1998 study in Planta Medica, researchers gave volunteers curcumin alone and then curcumin with piperine, and measured a dramatic increase in how much reached the blood.

Two honest caveats

The human portion of that study was small. It involved a handful of volunteers taking a single large dose. It is a real result and it is the foundation of an entire product category, but it is not a large trial.

The dose in the study was higher than what most products use. The research used around 20 mg of piperine. The industry standard in turmeric supplements is 5 mg of a standardised black pepper extract, which is roughly a quarter of that. Whether the effect scales down proportionally has not been established.

So the mechanism is real, the direction is right, and the exact size of the benefit at typical retail doses is less certain than the marketing suggests. That is still a far stronger case than most supplement pairings can make.

The other approach: lipid and phytosome delivery

Rather than slowing curcumin's breakdown, several branded ingredients change how it is carried, by binding it to fats, phospholipids or fine particles. You will see names like phytosome complexes, colloidal formulations and micronised curcumin.

These generally show better absorption than plain curcumin. Two things worth knowing. Most of the supporting research comes from the companies that sell the ingredient, and the comparison is usually against unenhanced curcumin rather than against piperine, so cross comparisons between technologies are hard to make.

How much should you take?

There is no official daily requirement for curcuminoids, so dosing follows the trials.

Most positive research on joint comfort used somewhere between 500 mg and 1,500 mg of curcuminoids per day, usually split into two doses, taken with food, for eight to twelve weeks. Note that this is curcuminoids, not turmeric powder.

Two practical points that follow from the arithmetic above.

Most retail products deliver far less than the trial doses. If a product provides 100 mg of curcuminoids a day, it is not delivering what the studies delivered, regardless of what the front label says.

Absorption enhancement is not the same as dose equivalence. Piperine raises how much of a given dose reaches your blood. It does not turn 100 mg into 1,000 mg. Both numbers matter and brands tend to talk about only one of them.

Take it with a meal containing some fat. Curcumin is fat soluble and an empty stomach is the worst case for absorption.

Turmeric and your liver: the part almost no buying guide covers

This section is uncomfortable for a company that sells turmeric. We are including it because you will not easily find it elsewhere, and because it changes how some people should approach this supplement.

Turmeric has gone from being considered essentially harmless to being a well documented cause of clinically apparent liver injury. The NIH LiverTox database now assigns it their highest likelihood category. In 2022, the US Drug Induced Liver Injury Network published a series of ten cases under the heading of a growing problem.

What the picture actually looks like

  • How many: several dozen published cases, against a population using turmeric in the tens of millions. The absolute risk is low. It is not zero.
  • When: usually one to four months after starting, sometimes as long as eight.
  • What kind: hepatocellular in about 90 percent of cases, with liver enzymes often rising above 1,000.
  • Recovery: most cases resolve within one to three months of stopping. Severe outcomes are rare but have included deaths.

The genetic finding, which is the most useful part

More than 70 percent of people who developed turmeric related liver injury carried a specific immune gene variant called HLA-B*35:01, compared with only 10 to 15 percent of the general population.

That is a striking difference, and it reframes the whole risk. This does not look like a dose problem that affects everyone equally. It looks like an uncommon reaction in a genetically susceptible minority. Most people will never have an issue. A small number are far more vulnerable, and there is currently no routine test to identify them in advance.

The part that concerns us about our own category

Stop taking turmeric and contact a doctor if you notice yellowing of the eyes or skin, unusually dark urine, pale stools, persistent nausea or loss of appetite, pain under your right ribs, or unexplained fatigue. These usually appear one to four months after starting. Most cases resolve once the supplement is stopped, and stopping early is what keeps them mild.

Lead in turmeric: a real and specific problem

Turmeric's colour is its selling point, and that has created a contamination problem you should know about.

Lead chromate, an industrial pigment, has been added to turmeric to brighten dull roots and make them look higher quality. Research from Stanford documented the practice across South Asia and traced elevated blood lead levels in Bangladesh back to adulterated turmeric. Enforcement action there has reduced it substantially, but the underlying economic incentive has not disappeared.

Lead is a neurotoxin with no safe level of exposure, and it is especially harmful to children and during pregnancy.

This is why third party heavy metal testing matters more for turmeric than for almost any other supplement. Not organic certification, which does not test for lead. Not "made in the USA", which describes where capsules were filled rather than where the root was grown. An actual independent laboratory test of the finished product.

Who should be careful with turmeric

Turmeric as a spice in food is fine for almost everyone. Concentrated supplements are a different proposition.

  • Blood thinners. Curcumin may affect clotting. If you take warfarin, clopidogrel, apixaban or similar, speak to your prescriber first.
  • Gallbladder disease or gallstones. Turmeric stimulates bile flow, which is unhelpful if there is a blockage. Avoid.
  • Liver disease, or a history of liver problems. See the section above.
  • Iron deficiency or anaemia. Curcumin can bind iron and reduce its absorption. Separate it from iron rich meals and iron supplements.
  • Pregnancy and breastfeeding. Supplemental doses have not been established as safe. Food amounts are fine.
  • Surgery. Stop two weeks beforehand because of the clotting effect.
  • Acid reflux or a sensitive stomach. High doses cause digestive upset in some people.
  • Kidney stones. Turmeric contains oxalates. Worth discussing with your provider if you are prone to them.

What turmeric is actually studied for

Most of the credible human research on curcuminoids concerns joint comfort and mobility, and markers of inflammation. Trials in this area generally ran eight to twelve weeks at the doses described above and reported improvements in self reported comfort and function.

Be aware of the pattern that runs through this literature: many trials are small, a good number are funded or supplied by the company selling the specific branded extract being tested, and results across different formulations are not directly comparable.

Turmeric is a supplement that supports comfort and everyday mobility. It is not a treatment for any disease, and any product or article telling you otherwise is making a claim it is not allowed to make. The label checklist

  1. Does it state a curcuminoid percentage? If not, you cannot calculate anything. This is the most important line on the panel.
  2. Does it separate turmeric root from standardised extract? Many products contain both. Only the extract portion carries meaningful curcuminoid content.
  3. Is there an absorption aid? Piperine, a phytosome, or a lipid delivery system. Plain curcumin absorbs poorly.
  4. Is it tested for heavy metals by an independent lab? For turmeric specifically, this is not optional.
  5. Is the serving one capsule or two? A 60 capsule bottle can be a 30 day or a 60 day supply.

  7. Does the claim match what turmeric is allowed to claim? Anything promising to treat a condition is a warning about the whole brand.
